Frequently Asked Questions

Ergo Amy Physical Therapy is a hybrid practice.  This means we offer a mixture of cash based services and also insurance based services.  We are an in-network provider with Medicare.  For all other insurance companies, we are an out-of-network provider.  This includes Medicare Advantage plans and all other commercial insurers.  If you have out-of-network benefits with your health insurance, as a courtesy, we will attempt to submit to your health insurance for reimbursement for your care.  If we are unsuccessful with insurance submission, we will provide you with all of the documentation needed to turn into your insurance company for reimbursement. Therefore payment may be due at the time of service.

If your deductible is unmet, an estimated payment is collected at the time of service.  We take cash, check, credit cards and HSA/FSA cards. When able, a courtesy bill will be submitted to your insurance company.  Otherwise, a superbill will be provided, that you may choose to use to submit to your insurance company for potential reimbursement depending upon your individual plan.   

We ask that you provide at least a 24 hour notice if it becomes necessary to cancel or change an appointment. If adequate notice is not received or in the event of a "no-show", you will be charged a $25 cancellation fee.

Ergo Amy Physical Therapy is located in south central Pennsylvania and covers the counties of: Adams, Cumberland, Dauphin, Franklin, Lancaster, Lebanon, Perry, York.  Referrals in other counties will be accepted on a case by case basis.  Virtual services are available via telehealth/video visits throughout the entire state of Pennsylvania.
